The survey document on this website is the same survey mailed to residents, with one exception: the name and address at the end of the survey are required before the form will successfully submit.
This ensures that multiples of the same survey are not accidentally submitted and counted, creating inaccurate results that don’t reflect the true composition of the community.

Please NOTE that the survey is an Adobe Acrobat document. While it is the most universal document type, it does not work with some devices, and requires Adobe Acrobat Reader to open the PDF form.
